Please complete the information below that is relevant to your project. If you need to provide more information than the space allows, please use attachments. Please do not exceed five pages not including the [Budget Form] materials. We strongly recommend you have your mentor review your application prior to submission to help optimize the alignment between the proposal and Lisle's missions. If you have questions about this requirement and/or do not know how to find one, please email Executive Director [mark.kinney@utoledo.edu].

Please use the amount which is necessary to describe your project well. The quality of the information is used to determine eligibility for consideration and evaluation of the application for funding. If you prefer, or should the form not provide enough space, please complete the [Microsoft Word version of the application].

Be sure to also submit the Lisle [Budget Form] materials. as well as this application form.
